By submitting this application, I/we, the undersigned student or parent/guardian of student, understand and agree to the following:

2. Use of Information: I/we consent to the use of the information provided in this application for the purpose of determining eligibility, awarding scholarships, and for internal reporting and statistical purposes.
3. Confidentiality: I/we understand that the information provided in this application will be kept confidential and will only be shared with authorized personnel involved in the application process.
4. I/we authorize the release of information provided in this application to scholarship donors, if required for the awarding of specific funds.
5. Right to Withdraw: I/we understand that I/we have the right to withdraw this consent at any time by providing written notice to the Advancement Office.
